Pursuant to 5ILCS 140/3/.1, the requestor must state whether the requested documents will be used for commercial purposes. The requestor will be notified as soon as the documents are available for inspection and/or copying.
Illinois state law does not require you to submit your request on a standard form, but the request needs to be in writing. The letter must include the information you are requesting, your name, address, date, and daytime phone number so the Shields Township Highway Department can contact you if there are any questions.
